    1. A brief type written Autobiography telling us why you believe you should be considered for this scholarship award.

    2. A listing of School Activities: Sports, Clubs, Offices you have held, Honors, and Other Extracurricular Activities or Events in      which you have been involved.
    3. A description of any Volunteer Work you have done for your School, Community and your Church.
    4. A listing of any paid Work experience.
